How to raise a concern
You can speak with Jared or contact Metamorph using the contact form. Let us know what happened, what you’re concerned about and what would help.
You’re welcome to have a family member, trusted person or advocate help you. You can also give feedback anonymously, although this may limit our ability to investigate or respond.
What happens next
We’ll listen, acknowledge your concern and discuss the next steps with you. We’ll look into what happened, keep you informed and explain the outcome, including any action we’ll take.
If your concern involves Jared, or you would prefer to speak with someone independent, you can contact the NDIS Commission directly.
Your support, your rights
Raising a concern will not result in punishment or unfair treatment. We’ll handle your information sensitively and explain when it needs to be shared to address the issue or meet a legal obligation.
Read our privacy information for information about how we’re planning to handle personal information.
Independent help
You can raise a concern about NDIS supports or services with the NDIS Quality and Safeguards Commission. You do not have to complain to Metamorph first.
